Hi, my name is Yasmin. I’m one of the hospice at home nurse practitioners here at Severn Hospice. Each year, we work with over 1000 families to provide complex symptom management and psychological support, allowing families to remain at home at the end of their lives. My job here at Severn Hospice is a real privilege. I’ve worked with many special families to support them to stay at home or to meet their needs as they wish, whether that be a long standing married couple sharing their final moments or supporting a young child to have a final hug with mum or dad. It’s a real privilege to be able to be in their moments and to be with people when they most need us. My job would not be possible without the help and support of people who decide to leave a gift in their will to Severn Hospice. We are proud of our unique services here and the way that we are able to support families in such an individualised way, and that’s what Severn Hospice care is to me.
